Our team completely understands how important end of life care can be to the individual who needs it and the family members involved. If it is someone’s wish to pass away at home, we work to discharge them from hospital as quickly and safely as possible. We work to make the process as easy as we can for everyone involved.

When at home, our priority is to make the patient comfortable and give them any help that we can. If the family is involved, we may not need to intervene as much as there are not as many responsibilities. However, if the client’s family is not present, our responsibility is much greater, and more help will be required.

We will do everything possible to make this difficult time as easy as it can be. Our role is to monitor the individual, help them as much as we can and be there for them.

Throughout the end of life care, we will work with the hospice team to make sure we are doing everything we can possibly do to make them as comfortable and relaxed as possible.
